How to complete a procedure?

Appointment Request

To request an appointment, please contact our Call Center line (604) 265 85 84 during our service hours:

Monday to Friday: 7:00 a.m. - 6:00 p.m.
Saturdays: 7:00 a.m. - 1:00 p.m.

On the day of your appointment, please arrive 30 minutes early to the admissions area of the assigned office. Do not forget to bring:

Identification document.
Service authorization (if applicable).
Medical history and test instructions.

Document Verification for Traffic Accidents

Before requesting your appointment, please go to the authorizations office at our main location with the following documents:

Initial care: Copy of the emergency report with FURIPS.
Medical history: From all facilities where you received care.
Medical certificate: Issued by your treating physician, with signature and registration number, stating that the injuries were caused by a traffic accident.
Billing statements: From the healthcare institutions (IPS) where you were treated.
Referral: For the requested service.
Optional documents: Sketch or police report.
Identification: Copies of the patient’s ID, SOAT, driver’s ID, vehicle registration card, and driver’s license.

Once the documentation is verified and approved, you may request your appointment through our Call Center line.

Medical Record Request

According to Resolution 1995 of 1999, the medical record is a private and confidential document. It can only be accessed by third parties with the patient’s authorization or in cases provided by law.

To request it, you must meet the following requirements:

If you are the patient: Present your original ID document.
If you are a family member or authorized third party:
  • Original ID document.
  • Copy of the patient’s ID.
  • Authorization letter signed by the patient.
If you are the parent or legal guardian of a minor:
  • Original ID document.
  • Copy of the minor’s ID.
If you are a family member of a deceased patient:
  • Original ID document.
  • Civil registry (birth, marriage, or sworn statement) to prove kinship.
  • Death certificate.
  • Signed request letter stating reasons and confidentiality commitment.
If the patient is of legal age and incapacitated:
  • Original ID document.
  • Civil registry (birth, marriage, or sworn statement) to prove kinship.
  • Medical certificate showing the patient’s health status.
  • Signed request letter stating reasons and confidentiality commitment.
